{"id":1348,"date":"2026-06-22T15:28:10","date_gmt":"2026-06-22T13:28:10","guid":{"rendered":"https:\/\/www.asd-agence.fr\/?p=1348"},"modified":"2026-06-22T16:16:05","modified_gmt":"2026-06-22T14:16:05","slug":"parcours-de-soins-en-partenariat-avec-le-ssiad-de-creutzwald","status":"publish","type":"post","link":"https:\/\/www.asd-agence.fr\/?p=1348","title":{"rendered":"Parcours de soins en partenariat avec le SSIAD de CREUTZWALD"},"content":{"rendered":"<p>Pour les b\u00e9n\u00e9ficiaires de la circonscription de Creutzwald, il vous est d\u00e9sormais proposer une prise en charge privil\u00e9gi\u00e9e avec un maintien\u00a0 \u00e0 domicile complet, gr\u00e2ce \u00e0 notre partenariat avec le SSIAD de Creutzwald.<\/p>\n<p>En compl\u00e9ment des prestations d\u00e9j\u00e0 propos\u00e9es par ASD (Aide et Service \u00e0 Domicile), vous avez la possibilit\u00e9 de contacter le SSIAD (Service de Soins Infirmiers \u00e0 Domicile) de Creutzwald qui couvre les soins d\u2019hygi\u00e8ne, les soins relevant des comp\u00e9tences des aides soignant(e)s et les soins infirmiers via son r\u00e9seau d\u2019Infirmier(e)s Dipl\u00f4m\u00e9(e)s d\u2019Etat Lib\u00e9ral(e)s.<\/p>\n<p>Ce partenariat entre nos deux \u00e9tablissements vous permet une prise en charge globale, privil\u00e9gi\u00e9e et coordonn\u00e9e de l\u2019ensemble de vos besoins. Les intervenants \u00e0 domicile \u00e9valuent votre autonomie et votre besoin d\u2019aide et de soutien tout au long de votre prise en charge.
